Message from MCBDDS - Please Read:

Welcome to the Service Request Portal for Montgomery County Board of Developmental Disabilities Services (MCBDDS). This software allows certified providers to view and respond to referrals for individuals in Montgomery County who are in need of services. In order to access this information, you must be certified by the Ohio Department of Developmental Disabilities (DODD), be in good standing and be interested in providing services in Montgomery County. 

If you have not registered for access to the Service Request Portal, click on the link to begin the registration process https://servicerequest.mcbdds.org/provider_register.aspx

Registration must be approved by the administrator before you can access the portal. Please allow 24-48 hours for activation of your account.

Your account may be deactivated if there is no activity in 60 days of your last log in. You will need to contact the administrator for a reset.

It is required that you as a provider maintain your current contact information with DODD and MCBDDS as this is essential as a business function of providers.
